Important Information
Please review the following details about our insurance program.
Primary vs. Secondary Coverage
Team insurance typically acts as secondary coverage, meaning your family's primary health insurance is billed first. The team policy covers eligible expenses not paid by your primary insurance, including deductibles and copays related to covered injuries.
Filing a Claim
If your child is injured during a team activity:
  • Seek immediate medical attention as needed
  • Notify the coaching staff within 24 hours of the injury
  • Request a claim form from the team administrator
  • Submit the completed form along with medical documentation within 30 days
What Is Not Covered
The team insurance policy generally does not cover:
  • Injuries during unofficial or unsanctioned activities
  • Pre-existing conditions or injuries
  • Individual training not organized by the team
  • Injuries sustained outside of official team events
Required Documentation
All players must have the following on file before participating in team activities: a signed medical waiver, proof of primary health insurance, an up-to-date emergency contact form, and any relevant medical history or allergy information.
